About Us and Contact Information
My father had suffered from dementia and wanted to stay at home. My family wanted to provide him with the best care and quality of life. We developed the care tools on this site to help us help him. They are available to help you.
Vision: Maximum quality of life at home for those impacted by chronic disease
Mission: Enable the best care at home for as long as possible
Strategy: Increase caregiving capacity by being well organized and sharing the care
Approach: Use care tools which enable well-organized, needs-focused, person-centered and team-based care
